Renewal of JO Contracts

To avoid delays in the processing of salaries, requests for renewal of all Job Order contracts for July to December 2013 must be received by the PHRMO on or before Friday, June 14, 2013. PHRMO will check all requirements and shall forward them to PGO for approval. All requests must include:

  1. Written request for renewal of Job Order contract addressed to Governor Victor A. Yap from the Department Head/ Chief of Office
  2. Job Descriptions (per employee), indicate time required to complete each task 
  3. Accomplishment Reports for January - May 2013, consolidated per month
  4. Personal Data Sheet (Form 212)
  5. Performance Evaluation Report for January - May 2013
  6. Photocopy of previous plantilla

Job Descriptions must be submitted to the PHRMO by Friday, June 7, 2013 at 9:00 a.m. Requests for rehiring with increase should be separate from requests for rehiring with no increase. Do note that only requests approved by the Governor will be processed and renewed.

Updates--
  • As per instruction from the Provincial Administrator and noted by the Governor, all Job Order employees are required to submit their Employee Output Reports every Tuesday (of the preceding week) using the new EOR format.
  • To celebrate International Women's Month, a CSC-accredited Stress Management in the Workplace seminar for select female employees is slated on Monday, March 18, 2013 at the SP Hall. Interested employees should coordinate with their respective Chiefs of-Office for proper nomination. All participants are requested to wear a pair of jogging pants and a white t-shirt. :)

Career Service Examinations - 2013

QUALIFICATIONS
Career Service Professional and Sub-professional examinations are open to applicants regardless of educational attainment. Applicants for both levels must:
  • Be Filipino citizens, at least 18 years old, and of good moral character;
  • Have no criminal record or have not been convicted by final judgment of an offense or crime involving moral turpitude;
  • Have not been dishonorably discharged from military service or dismissed for cause from any civilian position in the government; and
  • Have not taken the same level of career service examination within three (3) months from last examination taken.
REQUIREMENTS
  1. Fully accomplished Application Form (CS Form No. 100 (Revised November 2012)).  The spaces for “Signature of Applicant” and “Right Thumbmark” on the form should be left blank.  These shall be accomplished in the presence of the CSC processor.
  2. Original and photocopy of any valid ID containing applicant’s clear picture, date of birth, signature, and signature of the authorized head of the issuing agency such as Driver’s License, SSS ID, GSIS ID, Philhealth ID, current Company/Office ID, current School ID, Postal ID, BIR ID, Barangay ID, Voter’s ID, Valid Passport, or Police Clearance (Note: Any other ID not included in the list shall NOT be accepted) -- IDs with lacking information, particularly the applicant’s date of birth, should be supported by NSO-issued Birth Certificate or Birth Certificate authenticated/issued by the Local Civil Registrar (LCR).
  3. Examination fee of Php 500.00 for the Paper and Pencil Test (PPT)
  4. Four (4) copies of identical I.D. pictures with specifications, as follows:
a. Philippine Passport size (4.5 cm x 3.5 cm  or  1.8 inches x 1.4 inches)
  b. Colored, with white background
  c. Taken within three (3) months prior to filing of application
  d. Printed on good quality photo paper
  e. In standard close-up shot (from shoulder level up with the head and face occupying at least 80% of the picture and with the name tag positioned at approximately 1 inch or 2.54 cm below the chin)
  f. In bare face (with no eyeglasses or any accessories that may cover the facial features)
  g. Showing left and right ears
  h. Taken in full-face view directly facing the camera
  i. With neutral facial expression, and both eyes open
  j. With full and handwritten (not computer-generated) name tag in the format: First Name, Middle Initial, Last Name, and Extension Name (if any), and signature over printed name

SCOPE OF EXAMINATION
Professional Level
In English and Filipino: vocabulary, grammar and correct usage, paragraph organization, reading comprehension, analogy, logic, and numerical reasoning.
Subprofessional Level
In English and Filipino: vocabulary, grammar and correct usage, paragraph organization, reading comprehension, clerical operations, and numerical reasoning.

Both levels of the examination will have general information questions on Philippine Constitution; Code of Conduct and Ethical Standards for Public Officials and Employees (R. A. 6713); peace and human rights issues and concepts; and environment management and protection.

SCHEDULE OF EXAMINATIONS
First Exam:
  • Date: April 14, 2013
  • Deadline of application: February 28, 2013
Second Exam:
  • Date: October 13, 2013
  • Date for Filing of Application: April 15, 2013 to August 30, 2013
